How to write soap notes is not hard when you understand what SOAP stands for, so let’s start there. SOAP – Subjective, Objective, Assessement, Plan. Now that you have an understanding of SOAP, let’s overview how to write these notes. You want to observe the person and note subjective observations such as, John looked stressed and irritated. Then you want to write your objective notes such as, John’s skin was flushed and his temperature was 101.4. Next, you will write your assessment of what you diagnose as the issue such as, John has the flu.
